Description Hospital@Home Advanced Practice Provider (NP/PA) This role will offer acute-level care to patients who are able to remain at home. The provider will work with hospital providers to determine eligibility, work with multidisciplinary teams to determine care delivery, and work with hospital administration for quality improvement.
