JOB DESCRIPTION: This position will serve as the Care Manager for At-Risk Childrenfor AppHealthCare primarily focused in Alleghany and AsheCounties, as part of the Clinical Services team. The positionworks closely with the NC Division of Public Health and NCDivision of Children and Family Wellbeing, as well as theAppHealthCare care management team. The purpose of CMARCis to provide a set of care management services for at-risk childrenages 0-5 years.
