Job Duties:• Under the supervision of the Children and Youth with Special Healthcare Needs (CYSHCN) Coordinator, provides culturally competent social work services for CYSHCN clients and their families. Our social workers provide psychosocial assessments, brief counseling and risk management guidance, education, referrals, and case management/care coordination services directed toward decreasing social, emotional and/or environmental risks which affect health, social functioning and barriers to appropriate health care.• Primary referrals will be from the CYSHCN program, but the social worker may also be cross trained to provide social work services to other programs within the agency.• Serves as the back-up
