MDS/Care Plan Coordinator
Main Purpose of Job The MDS/Care Plan Coordinator will ensure all State and Federal required documents are completed accurately, timely and are submitted as mandated by law or standards. This position acts as a resource and nursing support to the Interdisciplinary Team to ensure appropriate admission, placement and discharge of patients as well as provide documentation as required by payers. MDS coordinators are responsible for the completion of time sensitive material, such as the MDS, Care Plans, ARDS, RAI and conduct care planning for the residents admitted at WMHC.
