Physician Specialist
The Physician Specialist will work with the multidisciplinary team and partner organizations to provide transitional healthcare until a patient has stabilized enough to transition to theconventional system of care (primary care and/or a community mental health clinic or ICMprogram) or has been placed in a higher level of care. Services are provided in office settings (MXM open access and urgent care) and outreachsettings such as streets and parks, homeless shelters, harm reduction sites, and congregate and non-congregate navigation centers.
