Purpose of Position: The Care Management For At-Risk Children is a population health program that is responsive to the varying needs of children and their families. The Social Worker is to collaborate and coordinate with families to ensure identification of and access to preventive, specialized and supportive services for themselves and their children. Clients served are children age's birth to five years of age who meet program identified risk criteria. They may be at risk for developmental delay, chronic illness, toxic stress and social-emotional disorder or diagnosed with such problems.
