Under the direction of the Supervising Health Care Evaluator, performs surveys and re-surveys to include evaluation of pharmaceutical services of Long-Term Care Facilities, Acute Care facilities and other health care facilities as part of a multi-disciplinary team of health professionals to determine whether health care facilities meet established requirements for State licensure and certification under Centers for Medicare and Medicaid Services.  Surveys and re-surveys will be conducted in Long Term Care Facilities, Acute Care Facilities, and other health care facilities throughout the state.  Prepares clear, accurate and informative reports of surveys of health care facilities.  Provides consultation to State surveyors, to officials of health care fa
