The Health Care Surveyor 2 in the Division of Home & Community Based Services Program will be responsible for routine surveillance and investigation of complaints and incidents of licensed home care services agencies, certified home health agencies, hospices, and Program of All-Inclusive Care for the Elderly (PACE) to ensure that care provided meets standards of practice and complies with State and federal statutes and regulations. The surveyor evaluates the care and services provided to individuals in their homes, assisted living facilities, adult homes, hospice residences, nursing homes and PACE programs/facilities.
