Nature of Work Provides nursing consultation and referral services to individuals, families, and target groups in the community. Works primarily within the Adult & Family Services Division with work assignments to include Long-Term Service and Support Assessment (MnChoices), Case Management, Care Coordination, and/or Personal Care Attendance (PCA) Assessments. Must be available to support the department during emergency situations.
