The Cumberland County Department of Social Services (DSS) and the Cumberland County Department of Public Health (CCDPH) have received funding to implement an initiative which focuses on primary prevention by utilizing a whole person/family approach and addressing social determinants of health (SDOH). The initiative includes multi-disciplinary Case Management and Care Coordination teams responsive to community referrals to provide care coordination and case management services to address social determinants of health, address social needs, improve health behaviors and health outcomes, improve health equity, and reduce health disparities, reduce preventable hospital stays, and prevent and reduce child abuse and neglect.
The Public Health Nurse II (PHN II) position is an in-home and community-based position. This position supports three multi-disciplinary Case Management and Care Coordination Teams. The position will provide limited clinical services to Cumberland County Department of Social Services (DSS) clients and Cumberland County Department of Health Department (CCDPH) patients receiving case management and care coordination services. The Public Health Nurse will be cross trained in child health, immunizations, maternity, family planning, STD, and adult health. This PHN II can provide follow up support and consultation for patients in these clinical areas within their scope of practice. The position provides primary, secondary and rehabilitative care resources to students in public schools, homes and communities. This nurse practices independently and demonstrates autonomous decision-making skills. The primary purposed is to promote health and prevent disease in the home and community.
Examples of Duties
Duties for this position include, but are not limited to;
Serves on multi-disciplinary Case Management and Care Coordination Team with Nutritionist, Social Worker, Health Educator and an Income Maintenance Case Worker to provide case management and care coordination services in-person (in home and office), by telephone or virtually.
Serves as a health care consultant for the Case Management and Care Coordination Team. Provides case management for students with complex health issues.
Performs in-depth nursing assessments. Collaborates with other health care team members in order to better assess the client's past, present and future needs and level of care.
Functions as a health resource for families and individuals including answering questions about medications, chronic diseases, etc., as appropriate.
Reports abuse and neglect to DSS.
Documents in department's Electronic Health Record and other systems as appropriate.
Collaborate with CCDPH staff including school health nurses, clinical staff, and WIC staff to assure families and individuals are receiving appropriate clinical services.
Employee is subject to be called upon anytime to help the agency respond to a public health emergency which may include, but is not limited to, outbreak investigations, staffing mass dispensing or immunization clinics, providing disaster relief, or other incidents (man-made or natural and intentional or non-intentional) as they arise, or any public health response requested by the Health Director.

Minimum Qualifications
Graduation from an accredited school of professional nursing and one year of professional nursing experience.
Necessary Special Qualifications - A current license to practice as a Registered Nurse in North Carolina by the N.C. Board of Nursing.
Degrees must be received from appropriately accredited universities.

Individuals employed in a local Health Department without a B.S. Degree which includes a Public Health Nursing rotation or a Master’s in Public Health, will be required to complete the “Introduction to Public Health Nursing” course within one year of employment as a condition of continued employment in accordance with Health Services Commission Rule.

Considerable knowledge of and ability to apply the principles and practices of public health. Knowledge of available resources and organizations and the ability to coordinate these as needed.

Ability to plan, coordinate, and oversee the work of others.
Ability to deal tactfully with others and to exercise good judgment in appraising situations and making decisions.
Ability to work in partnership with patients and with other service providers to elicit needed information and to maintain effective working relationship.
Ability to record accurately services rendered and to interpret and explain records, reports, and medical instructions.
Adequate computer skills to allow communication, patient record documentation, and accessing of information.
Must have or obtain and maintain a valid North Carolina driver's license with an acceptable driving record. Background check and negative drug test are required.

About Cumberland County Department of Public Health
The Department of Public Health provides an array of services to the citizens of the County.
Federal, State and County tax money fund the Health Department. Grants and fees for services generate additional funds. The Health Department is governed by the Cumberland County Board of Health, which is composed of up to eleven members appointed by the Board of Commissioners.
The Health Department was founded in 1911 as the Cumberland County/Fayetteville City Board of Sanitation and Health, but it had no paid employees. In 1923, a part-time physician and two nurses were hired as the first paid employees.
The Public Health Center is located at 1235 Ramsey Street and operates three satellite offices throughout the County.
