This position will serve as the Care Manager for At-Risk Children forAppHealthCare primarily focused in Watauga and Ashe Counties, as part of the Clinical Services team. The position works closely with the NC Division of Public Health and NC Division of Children and Family Wellbeing, as well as the AppHealthCare care management team. The purpose of CMARCis to provide a set of care management services for at-risk children ages 0-5 years. The program coordinates services between health care providers, community services, and family support programs.  Home visiting, community interventions, and meeting in providersettings are required.  This position reports to the AppHealthcare Care & Support Services Manager.  KNOWLEDGE, SKILLS,ABILITIES:
