Position OverviewThe Illinois Department of Public Health is seeking a highly motivated individual to conduct surveys,investigations and monitoring visits of facilities for persons with intellectual/developmentaldisabilities to determine compliance with State Licensure requirements and/or Federal Medicaidcertification regulations and prepares licensure and certification survey/investigation forms,memoranda and reports of findings. This individual conducts on-site monitoring visits at non-compliant facilities.Job Responsibilities1.Travels and conducts surveys of intermediate care facilities for individuals with intellectualdisabilities, facilities for medical complicated individuals with intellectual disabilities, andcommunity living facilities to determine
