Job Duties: • Under the supervision of the Children and Youth with Special Healthcare Needs (CYSHCN) Coordinator, provides culturally competent social work services for CYSHCN clients and their families. Our social workers provide psychosocial assessments, brief counseling and risk management guidance, education, referrals, and case management/care coordination services directed toward decreasing social, emotional and/or environmental risks which affect health, social functioning and barriers to appropriate health care. • Primary referrals will be from the CYSHCN program, but the social worker may also be cross trained to provide social work services to other programs within the agency. • Serves as the back-up to the Regional Social Worker Lead (RSWL), assisting as the secondary point-of-contact for regional SW issues/concerns, coverage, and consultation as needed. May also provide coverage for other social workers as needed. • Prepares written reports and clinical documentation according to agency, program, and professional practice guidelines. • Participates in agency and community groups to identify and develop resources for outreach. • As a valued member of the CYSCHN interdisciplinary team, the social worker serves as the mental health subject matter expert for CYSHCN services. • Participates in quality improvement activities and social work meetings with peers to improve the effectiveness of social work services and program activities.

Minimum and Additional Requirements
State Minimum Requirements: A bachelor's degree in social work from a program accredited by a nationally recognized accrediting body for social work programs, or from a social work program whose standards are at least equivalent to the minimum standards required by the nationally recognized accrediting body approved by the Board.
Agency Additional Requirements: Master’s degree in social work from a school of social work accredited by the Council on Social Work Education. Licensed as an LMSW or LISW in good standing by the SC Board of Social Work Examiners/SC LLR. DHEC employees are required to participate in disaster preparedness during hazardous weather or declaration of an emergency. This involves being subject to duty and or call on a 24-hour basis. You must possess a valid driver's license and have access to a reliable transportation. This position may require working early (before 8:30 am), evening (after 5:00 pm) and/or Saturday's. Ability to lift 30 pounds and stand for long periods of time.
Immunization Requirements: All new employees who are healthcare providers will be required to provide documentation of immunity or be immunized against measles, mumps, rubella, pertussis, varicella, Hepatitis B prior to beginning employment.
Public Health staff with job-specific risk of COVID-19 exposure who choose not to receive the COVID-19 vaccine or provide documentation of prior vaccination, or who are unable to be vaccinated due to medical contraindications or religious exemption, will be required to receive routine periodic COVID-19 testing by DHEC.
Vaccinated employees will not be required to receive periodic testing but may elect to be tested to monitor their health status.

Preferred Qualifications
1-2 years of experience as a licensed master’s social worker.
