A Bachelor’s degree in a related field and three years of experience in program coordination in a public health, health, health regulatory, or human services related program; OR an Associate's degree in a related field and five years of such experience; OR seven years of such experience. Experience and knowledge of HIV/AIDS services, programs, and/or systems for adults, children, youth and families. Experience with the provision of technical assistance to community-based organizations and/or health services providers. Experience monitoring and evaluating programs and quality improvement activities within a public health, health, government, or social service setting. Knowledge and familiarity of Health Homes and development of integrated health care service models. Knowledge and familiarity of NYS Medicaid Redesign Team (MRT) policy reform and/or managed care policies, and their relation to access to care/services for persons with HIV.
The Health Program Coordinator I (HPC I) position will be located in the AIDS Institute, Office of Medicaid Policy and Health Care Financing, Medicaid Policy and Programs unit. The HPC I will have responsibilities related to Medicaid programs within the AIDS Institute for persons living with or at risk of HIV/AIDS. The HPC I will have responsibilities related to quality improvement, evaluation, and standards development and implementation of Medicaid reform and emerging system changes for Health Homes, Designated AIDS Centers, AIDS Adult Day Health Care Program initiatives, and additional Medicaid activities.
Duties include, but are not limited to: contributing to the development of policies and standards related to the provision of Medicaid HIV program services for persons at risk for and living with HIV/AIDS and other chronic conditions; providing technical assistance to providers and to internal AIDS Institute, SDOH and other government agency staff; assessing the effectiveness and quality of programs and assessing provider compliance with program goals and policies; developing program reports and correspondence; coordinating and attending site visits; programmatic and fiscal oversight of contracts, and serving as a resource to advance the Ending the AIDS Epidemic initiative with Health Homes and care management/coordination programs. Other duties as assigned.
