Boston Children's Primary Care Center is looking to hire an energetic and compassionate individual to help patients with Autism Spectrum Disorder connect with community services. This person will be an integral part of the care team, acting as a link between the clinical team and the family to ensure patient needs are met. The Autism Care Navigator will be responsible for: - Working with families at key transition periods, particularly after a new diagnosis, aging out of Early Intervention, entering kindergarten, or transitioning to adulthood. The ACN will also conduct one-time consultations for other patients as needed.
- Conducting telehealth visits with families to help them connect with services and also provide education and support.
- Creating a resource toolkit for families, providers and staff, conducting trainings, and helping to coordinate care for patients with additional behavioral needs during medical visits.
- Receiving extensive training about the diagnosis of autism, treatments and available community resources during the first 6 months of the intervention, as well as ongoing professional development.
- Meeting with a member of the social work team weekly for ongoing supervision and to discuss cases.
- Attending monthly team meetings as well as Project Advisory Board and community partner meetings
The position calls for an individual with excellent organization and follow-through skills, the ability to work independently, and someone who is willing to collaborate closely with different members of the care team. This position requires interpersonal, communication and problem solving skills in order to work cooperatively and effectively with both families with complex medical and social situations, and an extensive care team. The ability to build and sustain strong relationships with families facing multiple challenges is required. To qualify, you must have: - A Bachelor's or Associate's degree
- Analytical skills and judgment to take appropriate action in varied circumstances and situations
- An ability to contribute to a supportive team environment with a diverse set of team members
- Strong attention to details, commitment to excellence, and a willingness to engage in creating a new role on a health care team
- The capability to use time efficiently, and the ability to prioritize and coordinate tasks appropriately
- Spanish language skills highly preferred.
- Flexibility in work schedule to be able to work some evening or weekend hours
