This Patient Flow and Capacity Center Triage Coordinator will be responsible for: - Collecting patient data, evaluating individual care requirements. Communicating with admitting, nursing and medical personnel to discuss and resolve placement issues/problems and plan and facilitate admission, transfer and discharge of patients.
- Responding to customer requests and resolving problems as needed. Evaluating service delivery systems and procedures and recommends changes to improve efficiency and effectiveness. Escalating issues up the chain of command as needed.
- Receiving and triaging calls as a member of a 24-hour transport communication center. Obtaining information from caller required to assess urgency of request and initiate appropriate response.
- Assisting in development and maintenance of systems and procedures supporting program operations. Working with nursing and interdisciplinary staff to identify, recommend and implement changes in pre-admission screening systems, procedures and technology which improve efficiency and responsiveness to patient/family needs. Monitoring and maintaining patient census forecasting system, instructing nursing personnel on preparation, collection and use of data. Collecting and maintaining program evaluation data and performing periodic analyses as requested.
- Identifying, communicating and/or responding to critical patient information affecting planning and provision of care and payor reimbursement. Identifying issues/problems regarding pending admissions, notifying designated personnel and/or initiating action according to specified guidelines and criteria. Informing patient care teams of patients' anticipated discharge date, destination and other related information.
- Maintaining knowledge of current nursing and health care trends and practices through participation in professional and industry organizations and attendance at conferences, workshops and other continuing education programs.
- Contributing to teamwork.
To qualify, you must have: - A Bachelor of Science in Nursing and nursing license in MA
- A minimum of 3-4 years of previous RN experience.
- Pediatric experience preferred.
- Work requires the analytical skills to collect information from diverse sources, apply professional principles in performing various analyses, and summarize the information and data in order to solve problems OR design relatively complex systems and programs.
- Work requires communication skills in order to effectively deal with conflicting views or issues and mediate fair solutions, or well-developed writing skills.
