The School of Public Health, Department of Health, Behavior and Society is currently seeking a motivated and experienced Case Manager to provide case management and care coordination services at SPARC, a harm-reduction focused drop-in center and outreach program for women and non-men in Baltimore MD. SPARC service provision has pivoted during times of COVID-19 and is largely centered on providing mobile services through South and Southwest Baltimore. This mobile service provision includes pop-up outreach and medical services; the delivery of harm reduction, hygiene and food supplies; and a mobile HIV/STI testing and Hepatitis C clinic.
In addition to conducting HIV/STI prevention, harm reduction education and distributing supplies, the outreach team provides resource lists and micro-counseling to further support the complex needs of our service users, many of whom experience comorbidities and chronic homelessness. Outreach activities take place in Pigtown, Carrol Park, Mount Clare, and Brooklyn of South Baltimore.
SPARC is also an authorized Overdose Response Program (ORP) and a Syringe Service Program (SSP), to best meet the needs of service users who use opioids. All staff are trained overdose response and reversal as well as drug user health, safer injection practices and trauma informed care.
Specific Duties and Responsibilities:
Minimum Qualifications: Bachelor's degree in social work or related discipline. One year of related human service experience, preferably in case management.
Preferred Qualifications: Prior field work experience preferred. Experience conducting outreach with diverse populations including sex workers and drug users strongly preferred. Successful candidates will undergo training sessions prior to working in the community.

During the Influenza ("the flu") season, as a condition of employment, The Johns Hopkins Institutions require all employees who provide ongoing services to patients or work in patient care or clinical care areas to have an annual influenza vaccination or possess an approved medical or religious exception. Failure to meet this requirement may result in termination of employment.
The pre-employment physical for positions in clinical areas, laboratories, working with research subjects, or involving community contact requires documentation of immune status against Rubella (German measles), Rubeola (Measles), Mumps, Varicella (chickenpox), Hepatitis B and documentation of having received the Tdap (Tetanus, diphtheria, pertussis) vaccination. This may include documentation of having two (2) MMR vaccines; two (2) Varicella vaccines; or antibody status to these diseases from laboratory testing. Blood tests for immunities to these diseases are ordinarily included in the pre-employment physical exam except for those employees who provide results of blood tests or immunization documentation from their own health care providers. Any vaccinations required for these diseases will be given at no cost in our Occupational Health office.
