Job Summary Under general supervision provides medication therapy management services and collaborates with other health care providers to optimize therapeutic outcomes. . Provides drug information to patients and health care workers as required, according to professional standards. Acts as a consultant for adverse drug reactions. Provides oversight of technical staff.

Knowledge, Skills & Abilities * Broad pharmacotherapy knowledge for understanding patient medication therapy needs, and identify/addressing medication transition issues. * Strong verbal and written communication skills, ability to interact with other members of the healthcare team and with pharmacy leadership.* Strong organizational skills.* Demonstrates accuracy, thoroughness, and attention to detail.* Ability to maintain accurate records and prepare reports.
Education * Bachelor's Degree (Required) Education and Experience1. Bachelor's Degree in Pharmacy required.2. PharmD preferred.3. PGY1 residency experience, preferred4. Five years of related experience required.
