Denver Health is currently recruiting for a full time Family Medicine Physician Assistant or Nurse Practitioner at our Montbello Family Health Center . Our Patient Centered Medical Home practice consists of a large group of physicians, advanced care providers, residents and fellows, as well as, a full complement of support, MA and nursing staff. The provider will have a minimum two years of practice experience as an advanced care provider preferred. The successful advanced care provider will have a strong desire to work in a Federally Qualified Health Center setting with a diverse group of patients and staff members. The Family Medicine Department at Denver Health consists of 40+ primary care physicians, 30+ advanced care providers, integrative behavioral health services and a full compliment of support staff. Under general supervision this provider will work with clinical staff/physician/manager to provide primary healthcare to patients with a focus on preventative and lifestyle modifications. Examines and diagnoses patients and develops treatment plans. As appropriate for clinical setting, provides treatment for illnesses, injury, emergencies, prenatal care, follow-up, screening, and testing. Performs testing, referral to physicians as needed, writes prescriptions, and performs procedures within the scope of expertise, protocols, and available equipment. The ideal candidate will be competent with birth control procedures including IUD and Nexplanon. Perform administrative support functions. Under the direction of a physician, admit, evaluate, diagnose, treat, and provide consultation to patients of all ages with acute and chronic illnesses, including major and complicated, and perform procedures learned in training. 2. Provide preventative and therapeutic care for patients of all ages. This includes preventative care, acute care, prenatal care, care of injured persons, management of chronic illnesses, and performance of procedures learned in training. 3. Performs privileged procedures as outlined in the Denver Health Advanced Practice Provider Ambulatory Care Clinical Privileges documentation. 4. Other duties and responsibilities as assigned. Knowledge, Skills & Abilities Has adequate knowledge and skill of all privileged procedures as outlined in the Denver Health Advanced Practice Provider Ambulatory Care Clinical Privileges documentation. Education * Master's Degree (Required) 1. Successful completion of a master's or post-master's degree from an NP program accredited by the Commission on the Collegiate of Nursing Education (CCNE) or the National League for Nursing Accrediting Commission (NLNAC), or successful completion of an Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) approved program (prior to January 2001 - Commission on Accreditation of Allied Health Education Programs) 2. Privileged by Denver Health Medical Office 3. One year related clinical experience preferred. Experience 2 years preferred as an advanced care provider in an outpatient primary care setting. Credentials * Nurse Practitioner (Required) 1. Current certification by the American Nurses Credentialing Center or an equivalent body, or current certification by the National Commission on Certification of Physician Assistants (NCCPA) 2. Valid license to practice nursing as an Advanced Practice Nurse or medicine as a Physician Assistant in the State of Colorado 1.
