This is a unique opportunity to work as team member on a pilot project with Boston Children's Hospital Accountable Care Organization (ACO) addressing social determinants of health for primary care teens and young adults, and their families. This social worker will work with an interdisciplinary team in the Adolescent and Young Adult Medicine Clinic and ACO staff to help ensure patient families have stable, healthy housing; access to nutrition resources; and receive all benefits and community resources available to them. This will primarily be a remote position, with occasional possible need to come on site. LICSW supervision provided The ACO Flexible Services Social Worker will be responsible for: - Identification and assessment of patients to determine eligibility for social support interventions
- Facilitation of referrals to community partners
- Case management and occasional short term crisis stabilization and support, including counseling and follow-up in regards to accomplishing goals of plan and discharge from program
- Coordination of nutrition education and resources, including providing direct nutrition education and skills development (e.g., food budgeting) counseling and providing support to enrollment in community nutrition classes
- Assistance with program development and establishment of program infrastructure
- Collaboration and relationship development/management with partner community agencies, including co-management of patients
- Clinical and project documentation, data collection, and organization of meetings
To qualify, you must have: - A Master's degree in Social Work
- LCSW in the state of MA, LICSW preferred
- Fluency in Spanish strongly preferred
- The ability to see patients both in-person, and virtually from home
- Interest and ability in both macro and clinical social work and public health
- Cognitive ability to observe, analyze and interpret general and technical data, (verbal and written) accurately
- Strong communication and interpersonal relationship skills with the ability to respond to events effectively
- Interest in working with multicultural patients with complex needs
- Ability to follow research protocols and procedures
- Expertise with building community partnerships, trauma-informed care, social determinants of health, housing policy and resources, and food security.
