Job Summary Under minimal supervision, plans, organizes and implements psychiatric social work treatment for clients and families. This work includes assessing the psychological needs of patients and families and carrying out interventions such as individual, group, and family therapy consistent with the spirit of evidence-based practices such as motivational interviewing and acceptance and commitment therapy. The work also includes identifying and connecting patients to resources to assist with their ongoing behavioral healthcare.
Denver Health's EIS Primary Care Clinic provides comprehensive outpatient services to patients with HIV at various clinics within the Ambulatory Care Services (ACS) system. The Clinical Social Worker is available to help patients and families to connect and provide resources to cope with the biopsychosocial stressors often associated with illness and/or injury. The Social Worker helps to identify those patients with complex medical problems, who are socially disadvantaged and present with multiple barriers to care. Successful integration of the Social Worker within ACS helps to improve the quality of care for the patient, assists the outpatient clinic with management of psychosocial issues/challenges and helps to promote an emphasis on patient empowerment and self-management goals.
Under minimal supervision, plans, organizes and implements social work treatment for patients and families. Assesses the biopsychosocial needs of patients and families and creates an individual plan of care, as it relates to the patient's medical care. Provides clinical interventions that help meet identified needs by utilizing personal, family and community resources. Provides short-term crisis intervention, supportive counseling, case consultation and care coordination services. Meets Ryan White Part C assessment and documentation programmatic requirements. Responsibilities
1.Conducts comprehensive biopsychosocial assessments that are thorough, individualized, objective and complete. 2.Provides intervention appropriate to the presenting problem. 3.Utilizes key case management components including assessment, care planning, coordination of care, monitoring, follow-up and documentation. 4.Evaluates emotional components, impact of health issues and assists patient to identify strengths, dysfunctional factors and to establish detailed and coordinated care plans. 5.Meets Ryan White Part C assessment and documentation programmatic requirements, addressing barriers to care and medication adherence 6.Provides care coordination throughout the continuum of care from case referral to case closure. Utilize/delegate activities to the Care Navigator as appropriate, for more administrative or time intensive tasks. 7.Applies knowledge of individual, group and family functioning to ensure appropriateness of treatment methods to assist patient in achieving care plan goals. 8.Is aware of and able to articulate the professional use of self in the relationship to maximize the patient's adjustment, enhance strengths and promote resolution of identified problems. 9.Demonstrates the ability to develop effective therapeutic interventions that are socio-culturally sensitive. 10.Routinely consults, negotiates, and coordinates with internal and external resources to assure collaborative efforts to maximize patient outcomes. 11.Demonstrates leadership in facilitating multi-disciplinary communications and patient care meetings (i.e. case conferences) and utilizes information to assess and reassess care needs. 12.Demonstrates and educates staff as to the scope of function of the Social Work role in the health care team. 13.Ensures patient involvement in care planning and goal setting with informed consent documented. 14.Complies with applicable laws and educates other staff as to legal issues related to Social Work practice and/or issues specific to the clinic Care Team. 15.Prepares written and electronic documentation as required. 16.Charting is thorough, timely and content reflects established qualitative standards. 17.Demonstrates knowledge and applicability of the principles of growth and development over the life span, as well as demonstrating the ability to assess the patient's status and interpreting appropriate cultural information of the patient(s) to whom care/service is being delivered/provided. 18.Demonstrates knowledge of major concepts principles and theories of normal and abnormal human development, and its application to specific age groups. 19.Utilizes knowledge of patient populations' cultural, religious, ethnic and social systems in interactions, care planning and education. 20.Utilizes working knowledge of medical terms, abbreviations and demonstrates this knowledge verbally and in writing. 21.Demonstrates comprehensive effective knowledge of programs, eligibility and skill in obtaining appropriate resource/services; is able to educate others, to identify gaps and suggest alternatives. 22.Educates patients and their systems regarding risk factors for psychosocial functioning, including but not limited to wellness and health education. 23.Demonstrates willingness to enter into learning in the utilization of supervision. 24.Recognizes growth needs and seeks opportunities to enhance learning skills/professional development. 25.Demonstrates ability to manage time and achieve established goals. 26.Advocates, as appropriate, in a professional manner. 27.Participates in learning of self and others through active participation in peer review process. 28.Participates in staff meetings, group supervision, staff development and assigned task groups and applies knowledge of groups and their tasks through appropriate participation. 29.For those assigned Supervision of an MSW Intern: Provides orientation, reviews new employee manual and educates as to rules and standards effecting performance prior to intern providing care. Monitors, reviews and co-signs all documentation to ensure compliance with CSW standards. Utilizes supervision to address learning needs, concerns and practice issues. Ability to respond to trauma or psychiatric emergency.
Knowledge, Skills and Abilities 1.Must have excellent interpersonal skills and ability to tolerate ambiguity. 2.Excellent written and oral communication skills. 3.Advanced knowledge of early childhood mental health, including typical child development and parallel process. 4.Ability to provide individual, child and family psychotherapy. 5.Knowledge of social emotional theories in education such as Pyramid and Conscious Discipline. 6.Experience with trauma-informed care and ability to apply to a large system/community. 7.Familiarity with systems interventions and assessment. 8.Knowledge of crisis intervention and ability to respond to trauma in both children and adults. 9.Knowledge of patient populations' cultural, religious, ethnic, and social systems in interactions, care planning, and education.Familiarity with community resources available to children and families. 10.Ability to maintain firm boundaries and confidentiality - to manage professional relationship including termination. 11.Ability to respond to feedback and incorporate into practice. 12.Comfort in providing group training and working collaboratively with others. 13.Bilingual Spanish and/or prior experience providing services to people with HIV and LGBT communities a plus. Computers and Technology 1.Proficiency with Microsoft Office 2.Epic Education Master's Degree in Social Work from a CSWE accredited college or university required.
Experience
Two years of full time post Master's Degree social work practice under the supervision of a licensed or equivalently qualified social worker or other DORA approved professional required. Credentials Basic Life Support (BLS/CPR) from American Heart Association (Required). Licensed Clinical Social Worker/State of Colorado or Licensed Social Worker/State of Colorado required.
