Children who spend significant time in the NICU after birth, due to preterm birth or complex medical problems, may benefit from special monitoring & interventions after hospital discharge. NICU GraDS provides ongoing medical & developmental evaluation and support for these children & their families. We provide consultation to families, pediatricians, & other medical subspecialists, regarding developmental & medical issues that are specific to infants at risk for ongoing medical & developmental problems.
The Nurse Practitioner will responsible for: - Providing outpatient medical and developmental support services to infants & young children ages 0-3 years who are considered high-risk due to preterm birth & associated health problems.
- Assessing, planning, implementing, & evaluating the care plan for a panel of patients. This includes evaluating individual health care status of patients by eliciting a patient history, performing a physical examination & age-appropriate developmental screening, assessing risk, and interpreting this information to establish a diagnosis.
- Developing & implementing a plan of care within defined guidelines for practice in consultation with attending physician and other members of the team.
Additionally, this role will dedicate 20% effort to interdisciplinary care integration and management for NICU GraDS patients & participate in the measurement of the impact of this role with support from the Department of Accountable Care and Clinical Integration. The individual will work with a multidisciplinary care team & a project team, to coordinate & track the care of patients with complex medical and developmental needs across different BCH subspecialties, primary care, & community agencies to promote value-based care initiatives and quality improvement.
To qualify, you must have: - MSN & current Nurse Practitioner license in MA.
- 3-5 years of pediatric Nurse Practitioner experience & critical care nursing experience is strongly preferred, but exceptional recent NP graduates will be considered with 3-5 years of transplant and/or critical care RN experience.
- The ability to work independently, must possess excellent time management skills with fine attention to detail.
- Excellent communication & collaborative skills, analytic skills to resolve issues of complex patient population.
